STUDENT-ATHLETE PAPERWORK
Please help us in providing quality care by completing the required paperwork that is needed by the Sports Medicine Staff. Below, you will find a link for the required forms that must be completed. Please identify the proper forms to complete depending on your status in school. After opening the forms, please utilize the ability to type your information into the forms before printing. When finished providing all requested information, please print off each form. After printing, be sure each form is signed appropriately (a parent/guardian signature is required on all forms for student-athletes under the age of 19). We suggest that you make copies of all of the forms to retain for your records. Please return the original completed forms PRIOR TO PARTICIPATION to the address below.

MEDICATION DOCUMENTATION
The NCAA requires that all athletes on stimulant medication, such as Adderal or Ritalin, for the treatment of ADD/ADHD provide adequate documentation of diagnosis and treatment to allow for a medical exemption. Stimulant medications are typically banned for use by NCAA athletes unless medical necessity is clearly documented by the host university. The Samford University Sports Medicine Department and the Department of Athletics is requesting additional information in order for your student- athlete to continue or begin their NCAA participation. This is critical for their participation in their sport. If you are seeing a physician who has prescribed any of these type of medications for you, please provide the following paperwork to that doctor and return the completed form to us at the address above.
